To enroll a student at Southwood High School the legal guardian must present the following documents:

1. proof of residency (current electric bill and a copy of lease agreement or mortgage lender documentation)

2. birth certificate

3. social security card

4. transfer form from previous school

5. most recent report card

6. most recent LEAP scores (summer school if necessary)

7. current health card from your child’s pediatrician or the Caddo Parish Health Unit:  1035 Creswell  676-5222)

If you cannot provide the required documents, you must call/visit:
